Using the Forms

Download fillable forms and save a local copy before entering responses. Printable forms can be completed using an appropriate PDF annotation tool or on paper. These are 2026 resources; the project team will issue cycle-specific instructions and dates for future cohorts.

T5 is a group discussion resource. T6 is a structured blank advisory-review template, rather than a reproduction of an individual board member’s completed feedback.

For current submission arrangements, contact the RAOP team. An online assessment submission area is planned.
